If you move within Germany:

Once you register your new apartment, you will automatically be deregistered from your old one. The registration office takes care of that for you. Please keep in mind that you have to register within 14 days after moving to a place.

If you leave Germany to live somewhere else:

You have to deregister from your German residence within latest 14 days after moving away (earliest 7 days before moving). The de-registration is effective as of your official move-out day. You usually have to fill a de-registration form, as well as present your passport. Please check your city registration office’s website for the details on the process and requirements.
